Health at Every Size® (HAES)

What is Health at Every Size® (HAES)?

Health at Every Size® (HAES) is a weight-inclusive and body-positive therapeutic approach that focuses on promoting health and well-being for our patients, rather than prescribing weight loss or pursuing an ideal body shape. HAES aims to foster body acceptance, encourage intuitive eating, and cultivate self-compassion, providing a supportive and nurturing environment for our patients to develop a healthier relationship with food, their bodies, and themselves. By challenging societal expectations and internalized beliefs about body size and shape, HAES helps our patients prioritize overall mental and physical health, independent of weight.

Our Approach to Health at Every Size® (HAES)

We understand that each individual’s needs and experiences are unique, which is why our HAES services are tailored to clients across all age groups. Our therapy sessions aim to foster body positivity, encourage intuitive eating, and promote self-compassion, providing a supportive and nurturing environment for personal growth and well-being.

Our Four-Step Process

  1. Assessment and Goal Setting: Our therapists collaborate with clients and their families to identify challenges related to body image, eating behaviors, and overall health. Together, we set realistic treatment goals and develop a personalized treatment plan aligned with HAES principles.
  2. Body Acceptance and Self-Compassion: We help clients explore and challenge societal expectations and internalized beliefs about body size and shape. Our therapists guide clients in cultivating body acceptance, self-compassion, and a more positive self-image.
  3. Intuitive Eating and Mindful Movement: Our therapists support clients in developing a healthier relationship with food and movement by embracing the principles of intuitive eating and encouraging enjoyable, sustainable physical activities that promote overall well-being.
  4. Review and Integration: Our therapists and clients regularly review progress, address any barriers to success, and integrate the learned skills and perspectives into daily life, fostering lasting change and a healthier relationship with food, body, and self.

Dialectical Behavior Therapy Enhanced(DBT-ED)

What is Dialectical Behavior Therapy For Eating Disorders (DBT-ED)?

DBT-ED integrates the core principles and techniques of DBT, including emotion regulation, distress tolerance, interpersonal effectiveness, and mindfulness, with specific strategies that target the dysfunctional attitudes, behaviors, and emotional issues associated with eating disorders.

In DBT-ED, individuals learn to identify and change problematic eating behaviors, manage difficult emotions without resorting to unhealthy eating behaviors, improve interpersonal relationships, and live more mindfully. It’s a comprehensive, evidence-based approach that supports individuals in achieving a healthier relationship with food and their bodies, and in improving their overall quality of life.

Benefits of DBT-ED Therapy

Dialectical Behavior Therapy for Eating Disorders (DBT-ED) offers several benefits for individuals struggling with eating disorders:

  1. Improved emotion regulation: DBT-ED provides individuals with tools to better manage and regulate their emotions, reducing the likelihood of turning to unhealthy eating behaviors as a coping mechanism.
  2. Reduced eating disorder behaviors: DBT-ED directly targets problematic eating behaviors, helping individuals to develop healthier eating patterns and to break the cycle of disordered eating.
  3. Enhanced interpersonal effectiveness: DBT-ED teaches individuals skills to improve their interpersonal relationships, which can often be strained by the impact of an eating disorder.
  4. Increased mindfulness: Through DBT-ED, individuals learn to practice mindfulness, which involves being fully present and engaged in the current moment. This can help to reduce preoccupation with food, weight, and body shape and to promote a healthier relationship with food and the body.
  5. Personalized treatment: DBT-ED is typically tailored to the individual’s specific needs and struggles, making it a flexible and personalized approach to treatment.
  6. Evidence-based approach: DBT-ED is grounded in research and has been shown to be effective in reducing symptoms of various eating disorders, improving overall mood, and enhancing quality of life.

Cognitive Behavioral Therapy-Enhanced (CBT-E)

What is Cognitive Behavioral Therapy-Enhanced (CBT-E)?

Cognitive Behavioral Therapy-Enhanced, or CBT-E is one of most effective treatments we have today for the treatment of eating disorders. It is a Transdiagnostic method for the full range of eating disorders – anorexia, bulimia nervosa, binge eating disorder, and related body image conditions. It a targeted, time-limited treatment that lasts for 20 – 40 Sessions depending on severity of symptoms.

It was developed as an outpatient treatment for adults by Christopher Fairburn, MD, at the Credo-Oxford Institute in the U.K. Dr. Riccardo Dalle Graves, MD is the developer of the adapted version of CBT-E for Adolescents that utilizes family support. International research has shown the success and efficacy of CBT-E.

An important study compared CBT-E with interpersonal therapy (IPT), also a leading treatment for adults with an eating disorder. At post-treatment, 66% of the CBT-E participants were in remission from their eating disorder, compared to 33% of the IPT participants. Over the follow-up period, the CBT-E recovery rate remained higher than IPT (69 versus 49 percent).

The Four Stages of CBT-E

Embarking on CBT-E, begins with a full evaluation and exploration of one’s eating problems .CBT-E is a protocol that helps to develop a highly personalized road map to recovery. Collaboratively, CBT-E therapists and client create a formulation of the individual’s unique eating and body image problems. Treatment progresses through four distinct stages, to effectively identify and tackle the underlying thoughts, beliefs, and behaviors that contribute to and sustain the eating disorder, such as food restriction or Binge/Purge Cycles. Treatment can include Exposure Work for “fear foods” , strategies for urges to binge or purge, and addressing underlying issues of negative body image, poor self-esteem, and clinical perfectionism.

Family-Based Treatment (FBT)

What is Family-Based Treatment (FBT)?

Around the world, Family Based Treatment is considered to be First-Line-of-Treatment for Child and Adolescent Eating Disorders, particularly for early onset Anorexia, Atypical Anorexia, Bulimia, and ARFID (Avoidant Restrictive Food Intake Disorder). FBT, or the Maudsley approach, is a Gold Standard treatment. FBT is a culturally-sensitive method that keeps a child in their family environment. In the FBT method, the parents play a major role in the re-nourishment of their starving child, under the guidance of the trained and certified Family Based Treatment practitioner and in medical collaboration with an outpatient Pediatrician or Adolescent Medicine Specialist. The child, adolescent, or young adult, and his or her parents, are carefully screened to be appropriate candidates for this protocol, conducted in the Least-Restrictive-Environment – your home!

FBT is a Culturally-Sensitive Treatment

At Resilience, Barbara Reese and her team practice the formal FBT protocol, including caregivers and siblings, with certification from the Training Institute of Child and Adolescent Eating Disorders. The FBT protocol, developed at the UK’s Maudsley Clinic, consists of around 20 sessions and differs from traditional eating disorder treatments that rely on insight and motivation. FBT acknowledges anosognosia, a condition where individuals don’t recognize their illness severity.

Parents don’t cause eating disorders but are crucial allies in recovery. FBT leverages parents’ knowledge and awareness to counteract the effects of anosognosia. Anorexia, a brain disorder, responds better to initial nourishment to restore brain function than to insight-driven treatments.

Hope-Full Nutrition

FBT is a a well-researched protocol. It is best when practiced adherently and adherent does not mean rigid. It means using interventions with the highest probability of success and not wasting precious time with interventions that are known to delay or interfere with recovery. Your FBT provider is an experienced therapist and family therapist who will follow your childs individual progression thru the 3 stages of FBT:

PHASE 1:

Refeeding/Weight Restoration

  • The family partners with an FBT Practitioner
  • Parents take full charge of “plating” meals/snacks, suspending calorie-burning exercise, and interrupting ED behaviors
  • Parental Teamwork, and role of other Caregivers/Siblings is established
  • A physician monitors for Outpatient Safety during Refeeding Process

PHASE 2:

Transitioning the Control of Eating Back to Child/Adolescent

  • Eating flexibility and eating independence emerges
  • Parental Supervision lessens when Adolescent achieves full weight restoration.
  • Exposure work is conducted to tackle “Fear Foods” and feared eating situations.
  • Physical activity may be gradually introduced here

PHASE 3:

Focus on Normal Adolescent Issues and Terminate Treatment

  • “State not Weight”, achieving full mental health recovery weight
  • Eating disorder thinking and behaviors begin to resolve
  • Flexibility and normalization in eating is established
  • Normal adolescent life becomes possible the with nourished brain/body
